$1500 Hiring Bonus available for applicants with active ASW,
AMFT, APCC, LCSW, LMFT or LPCC degrees, additional $1500 Hiring
Bonus for fluent Spanish We currently have 1 Clinician needed in
Napa and 1 Clinician needed for Yolo **NAPA OFFERS 10% SALARY
DIFFERENTIAL JOB SUMMARY The Clinician I functions as the lead case
coordinator, providing comprehensive, strength-based, culturally
competent, flexible, effective, quality mental health services,
including psychotherapeutic interventions, to eligible at-risk
youth, and families. The Clinician I provides services in
accordance with County contract requirements, Medi-Cal standards
and the agency's principles, including family voice and choice
provided in collaboration with those individuals and agencies
involved with the youth. Supports the Mission, Vision, and Values
of the agency. QUALIFICATIONS Education & Experience A Master's
degree in Social Work, Psychology, or Counseling; Registered as an
Associate Social Worker (ASW), Associate Marriage and Family
Therapist (AMFT), Associate Professional Clinical Counselor (APCC);
and At least 1 year of experience providing services to youth and
their families Skills & Requirements Prepared to work independently
with flexible hours that may include evenings, weekends and
holidays Proven documentation skills and intermediate level
computer skills including all MS Office programs Adhere to all
confidentiality requirements Verbal and written ability to
communicate effectively with colleagues, community partners,
consumers and all levels of management Ability to multi-task and
prioritize projects and assignments effectively Ability to work
cooperatively in a team environment within program/department and
agency wide Ability to work in a mobile, fast paced environment
Ability to use a laptop computer Fulfillment of TB test, background
check clearance, and any other mandatory State/Federal requirements
Valid California driver's license, proof of automobile insurance,
can maintain a driving record acceptable to the agency's insurance
carrier, if applicable to position Must provide and operate a
registered, properly insured automobile, if applicable to position
ESSENTIAL JOB FUNCTIONS Position Specific Functions as the lead
case coordinator, providing comprehensive, strength-based,
culturally competent, flexible, effective, quality mental health
services, including psychotherapeutic interventions, to eligible
at-risk youth, and families. Provides services based on clinical
need and in consultation with supervisor to assigned youth, and
families in the home, office, school, or other community based
settings. Coordinates and facilitates initial intake meeting with
consumers meeting agency and County timelines and expectations
Responsible for the overall assessment, development, and
implementation of Assessment Client Plans and Re-Assessment and
Re-Authorization Client Plans Provides and documents all Medi-Cal
eligible billable and non-billable services, including but not
limited to assessment, plan development, group session, collateral,
rehabilitation, In Home Based Services (IHBS) case management,
Intensive Care Coordination (ICC) and crisis intervention within
required timelines Ensures every family has an individualized
safety plan that maximizes the use of natural supports and
community based resources Coordinates care with any external
providers (i.e. psychiatry services, etc) Attends all staff
meetings, clinical supervision, and consultation meetings as
scheduled Provides services to youth involved in the Child Welfare
Services, Juvenile Justice, and Mental Health systems, meeting all
County requirements as listed in program scope Participates in the
agency on-call rotation Participates in utilization, accreditation,
peer review process as assigned Responsible for meeting all
outcomes as documented in Employee Development Plan Models and
communicates appropriate positive attitudes towards the agency's
Mission, Vision, and Values and program outcomes Works
collaboratively with all internal programs, departments, and staff
to provide support as needed Maintains positive relationships with
consumers and customers Coordinates with other services provided
under the Medi-Cal model, acting in collaboration with other
partners and ensuring documentation reflects standards that meet or
surpass the agency, County Children's Mental Health, and
Short-Doyle Medi-Cal standards Agency Specific Performs all duties
in a manner consistent with the principles and values of agency,
while adhering to applicable professional codes of ethics, the
agency's policies and procedures, contractor requirements, and
regulatory requirements. Model and communicate appropriate positive
attitudes toward the agency's Mission, Vision, and Values Work
collaboratively with all agency programs and staff to provide
support as needed Utilize and maintain calendar with all
work-related details in order to manage time effectively and share
calendar information with coworkers Participate in on-going
training to expand and develop professional skills Perform other
duties as necessary for the agency, as assigned Employment Type:
