Shifts (08:00 AM – 4:30 PM) Providence Santa Rosa Memorial Hospital
is seeking an experienced and compassionate RN Case Manager to join
our interdisciplinary care coordination team. This role is
essential in guiding patients through their hospital stay, ensuring
timely care transitions, and optimizing outcomes by collaborating
closely with providers, patients, families, and community
resources. Active RN license in California Minimum of 2 years of
case management or acute care nursing experience preferred
Knowledge of utilization review and discharge planning Established
