Job Description The Crox Group is seeking a local contract nurse RN Case Management for a local contract nursing job in Santa Rosa, California. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Duration: 12 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract SKILLS: - 2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment - required - clinician must have current work history within 2 years. - Computerized documentation, required. - Ability to organize and participate in family meetings - Participate in the development of the treatment plan. - Experience with HMOs, IPAs, etc. - Competent with arranging DME, SNF and ambulance transport.
