Job Description:
Salary: $33 - $35 / per hourMaxim Healthcare is looking for a
Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) who
assumes responsibility and accountability for the application of
the nursing process and the delivery of patient/client care. The
LVN/LPN is also responsible for providing services, as directed by
contract and/or program, while under the supervision of a
contract/program Registered Nurse or clinical designee. The LVN/LPN
provides service in accordance with Maxim's philosophy, standards
and policies.Essential Duties and Responsibilities:Utilizes the
nursing process to plan, implement, and evaluate patient/client
care and/or services provided to client(s) receiving contracted
services, as requiredOn each assignment, provides skilled nursing
care/services in accordance with prescribed ordersIdentifies and
documents changes in patient/client physiological and psycho-social
conditionNotifies Director/Supervisor, and contracted customer
designee if applicable, when:Physician notification is required in
order to obtain necessary orders for intervention(s), per state
regulationsChanges in patient/client condition have occurredAdheres
to Maxim policy, procedure and applicable protocolsIdentifies and
prioritizes actions based upon patient/client care requirements
and/or contracted customer requirementsEnsures that documentation
reflects patient/client collaboration including participation in
plan of care creation and maintenance, physician
correspondence/collaboration, expected patient/client outcomes,
nursing interventions employed, and other items as required by
contracted customerPerforms/provides interventions, treatments, and
medication administration, according to identified priorities, the
plan of care, facility policies and patient/client care outcome
standards as applicableMonitors and documents effectiveness of
interventions, treatments, and medicationsAssures patient/client
safety at all timesMonitors the safety and effectiveness of the
care/services environment and equipment used, including universal
precautionsExecutes consistent, effective interpersonal
communications and maintains positive, professional relationships
with patient/client and contracted customer stakeholdersAdheres to
the following professional practice standards:Nurse Practice

of Accrediting BodyState and Local LawsPatient Bill of
RightsPolicies and procedures of contracted customersAdheres to

requiredPerforms other duties as assigned/necessaryMinimum
Requirements:Graduate of an accredited practical nurse or
vocational nursing program preferredCurrently licensed as a
Licensed Practical Nurse or Licensed Vocational Nurse license in
specific state requiredComplies with accepted professional
standards and practice requiredOne year prior professional nursing
experience preferredWillingness to participate in skills
advancement training or novice nurse program if applicableCurrent
CPR, as applicable, for direct patient carePPD or chest x-ray if
applicableSuccessful completion of Maxim Orientation and Compliance
Training, and all other assigned trainings as applicable
requiredSuccessful completion of Maxim's Competency Program
requiredProficiency in the English language requiredCurrent Health
Certificate (per facility contract or state regulation)
requiredMust meet all federal, state, local, and contract
