Modules are required to be done before start of assignment. Modules
take on average 6 hours to complete.
Minimum of 2 years experience as a PT in an acute/ care/rehab
setting. Must be a licensed driver with an automobile that is
insured in accordance with state and/or agency requirements and is
good working order.
Required certs: Current California license as a PhysicalTherapist ,
BLS and NPI #
PT performing health re-visits
The service area is Yolo County.
HH requires a current driver's license & proof of automobile

This individual provides physical therapy services under the
direction of the Physical Therapist and
according to the physician's plan of care.
-Provides physical therapy services to patients/clients according
to a written physician's plan of care. This may include, but is not
-Assessing, evaluating, and updating
therapeutic/rehabilitative/functional status, and participation in
the development of the total plan of care.
-Evaluates environment. Assesses for muscle strength, mobility,
gait, ROM - potential for rehab. Directing physical therapy
-Instructing patients/clients and families/caregivers in the use
and care of therapeutic appliances. Determining priority needs for
-Reporting the patient's/client's reaction to treatment or changes
in condition to the physician.
-Initiates physical therapy program and instructs other personnel
and/or family/caregiver members in certain phases of physical
which they may work with a patient/client, as well as instructing
them as to the goals of the physical therapy program for the
participating in case conferences.
-May train patient/client in the use of prosthetic device.
-Supervising maker/ Health Aides when such personnel are
participating in patient's/client's rehabilitation regime.
-Identifies patient/client and family/caregiver needs for other
health services and refers as necessary.
-Prepares and submits clinical and progress summaries based on the
attainment of goals, observing, recording and reporting information
patient's condition to the attending physician and in the patient's
