The Case Manager is responsible for managing the complex process of efficiently coordinating an individual's movement through the continuum of care. This is accomplished through effective intervention and communication with the individual, their family, physician, caregivers and payors in assessing individual's needs, response to treatment, determining a target for length of stay based on medical diagnosis, and collaborating with other members of the health care team to develop and implement continued care plans based on the identified needs and goals of the individual. -#carecoordination

Minimum Requirements:Current licensure as a registered nurse in the State of California. -Current BLS from AHA The position requires a minimum of 3-5 years of nursing experience in an acute care facility or 2 years of Case Management experience in a related health care organization. - A combination of education and experience which provides knowledge, skills and abilities necessary for outpatient and inpatient resource management, including utilization review screening criteria, governmental, HMO and 3rd party payor regulations will be considered. -Additional Requirements:Ability to work as a team member to meet team goals and objectives. - -Work effectively under multiple demands and adjust priorities as circumstances require to meet needs of the department. - Maintain a high degree of accuracy and precision on a consistent basis. - Treat co-workers, physicians, patient's families, visitors and the public with dignity and respect. - Establish and maintain effective working relationships. - Maintain patient confidentiality. - -Physical Factors: Physically be able to stand and stoop. Able to walk and stand at least 50% of the shift. Good manual dexterity and visual and auditory acuity. Ability to sit for long periods of time at a computer.
