About us:We are a Federally Qualified Health Center (FQHC) and have
been providing care to Yolo County residents since 1972. Like other
community health centers, we provide care to anyone that walks
through our doors and are located in all major cities within Yolo
County (Davis, Woodland and West Sacramento) and provide services
throughout Yolo County. The community health center movement was
born out of the civil rights movement in the 1960s with the mission
to provide healthcare in communities where there are few or no

About the role of: Integrated Behavioral Health Specialist
(ASW):ASW (Associate Clinical Social Worker) independently provides
brief, solution-focused psychotherapy, and psychoeducation to
individuals who are being served by CommuniCare (Primary Care,
Dental, Perinatal) in an integrated and comprehensive care delivery
Provide in-person and telehealth services including assessment,
triage, crisis support, and individual and/or group therapy for
patients experiencing a range of mental health symptoms in a
trauma-informed and culturally responsive manner.
- Collaborate with primary care, dental, clinic staff, and Yolo
County Health and Human Services Mental Health to provide and
coordinate care, including substance use disorders and level of
- Ability to quickly and accurately attune to clients during Warm
Hand Offs from Primary Care and Dental Providers.
- Competently deliver therapy services to children and adults
with client goals being at the center of care.
- Independently manage complex caseload in back-to-back 30-minute
windows and complete administrative duties in fast-paced medical
- Effectively utilize measurement-based care practices to
determine client progress, wellness, and risk factors; collect
required departmental pre/mid/post health screenings (e.g., PHQ9).
- Complete clinical documentation within 48 hours of services in
accordance with working hours, all documentation must comply with
Medi-Cal, Medicare, Partnership, Beacon Health guidelines.
Participate in peer chart reviews and other quality improvement
- Available for in-person and video-based crisis, triage, and
consultation during primary care and substance use disorder
- Complies with established department policies and procedures,
objectives, quality assurance program, safety, environmental, and
patient service standards.
- Based on the individualized patient assessment, develops a
treatment plan of provision of services in collaboration with the
client and includes measurable goals.
- Identifies referral needs. If needed, makes referrals for more
intensive psychological or psychiatric services.
- Ability to address a wide range of mental health conditions,
utilize a variety of assessment tools, facilitate diagnostic
evaluations and treatment interventions to address client needs,
goals, and circumstances.
- Refers patients with apparent need for psychotropic medication
to primary care provider and/or psychiatrist where
- Participates in integrated system of care, staff meetings, case
review, consultation/supervision sessions and other forums as
- Conducts all work within generally accepted standards and
ethical guidelines of the social work or counseling professions.
Seeks out guidance from Associate Director of BH/BH Manager when
questions or concerns arise.
- Creates a supportive discharge process, in collaboration with
client and Primary Care providers.
- Other duties as assigned.Education/Experience Required:
Associate Clinical Social Worker (ASW) with current registration
with the California Board of Behavioral Sciences required. Must
have completed at least 2500 hours of clinical experience towards

Davis, Woodland). Able to proficiently engage and provide care to
patients by way of HIPAA complaint telehealth/video-based services
and fully participate in video-based meetings and trainings. Must
have ability to effectively use telephonic translation services
when client language preference dictates.
