Responsibilities
We're seeking candidates with the following skills and
strengths:
The Registered Nurse coordinates, supervises and implements patient
care. Observes and maintains records on patient care, condition,
reaction and progress. Conducts initial and ongoing assessment,
develops patient care plan, manages patient treatment, teaches
patient and family, identifies and corrects variances from care
plan and completes discharge planning. Administers prescribed
medications, changes dressings, cleans wounds, monitors patient
vital signs and provides patient and family education on treatment
plan and ongoing care needs. Obtains consults and referrals as
necessary.
Knowledge of current nursing care techniques, principles and
procedures. JCAHO, Title 22, Medicare regulations pertaining to
Home Health. Must have ability to: Work as a team member to meet
the department goals and objectives. Treat co-workers, patients,
physicians and family members with dignity and respect. Maintain
patient confidentiality. Communicate effectively with team members
in verbal and written form, using electronic clinical information
system. Establish and maintains a cooperative working relationship
with physicians, nursing staff, therapist, other hospital
departments, and outside agencies.
Maintain knowledge of changes governing Home Health Agencies in
accreditation, State and Federal Regulation, third party payors,
and patient care needs. Work a flexible schedule and be willing to
work overtime if needed for the provision of quality patient care.
Function autonomously.

WHC - Home Health care Program. This position will require travel
within Yolo, Sutter, Yuba and Solano Counties. The candidate will
be required to have a working vehicle, a valid drivers license and
proof of car insurance.
