Responsibilities
We're seeking candidates with the following skills and strengths:

- Maintain knowledge of changes governing Home Health Agencies in
accreditation, State and Federal Regulation, third party payors,
and patient care needs
- Enjoy working a flexible schedule and be willing to work
overtime, if needed, for the provision of quality patient care
- Enthusiasm about field nursing care, as our patients will span
throughout Yolo, Sutter, Yuba, and Solano Counties. -
- Knowledge of current nursing care techniques, principles and
procedures, The Joint Commission, Title 22, Medicare regulations
pertaining to Home Health. -The Registered Nurse coordinates,
supervises and implements patient care. Observes and maintains
records on patient care, condition, reaction and progress. Conducts
initial and ongoing assessment, develops patient care plan, manages
patient treatment, teaches patient and family, identifies and
corrects variances from care plan and completes discharge planning.
Administers prescribed medications, changes dressings, cleans
wounds, monitors patient vital signs and provides patient and
family education on treatment plan and ongoing care needs. Obtains
consults and referrals as necessary. -Knowledge of current nursing
care techniques, principles and procedures. JCAHO, Title 22,
Medicare regulations pertaining to Home Health. Must have ability
to: Work as a team member to meet the department goals and
objectives. Treat co-workers, patients, physicians and family
members with dignity and respect. Maintain patient confidentiality.
Communicate effectively with team members in verbal and written
form, using electronic clinical information system. Establish and
maintains a cooperative working relationship with physicians,
nursing staff, - -therapist, other hospital departments, and
outside agencies.Maintain knowledge of changes governing Home

schedule and be willing to work overtime if needed for the
provision of quality patient care. Function autonomously. The

candidate will be required to have a working vehicle, a valid
drivers license and proof of car insurance. -

Qualifications
Sign on bonus eligible up to 3kRequired:Two years of Med/Surg/Tele
or ICU nursing experience within the past 3 years, or one year Home
Health nursing experience within the past 3
years.Licensures:Current California RN license.CurrentBasic Life
Support (BLS) from American Heart Association
(AHA)CurrentCalifornia Driver's License, or out of state license
consistent with California DMV regulations. Must obtain California
DL within first 30 days of employment.Must have current auto
insurance and access to an operating motor vehicle.Preferred:Home
Health nursing experience preferredEducation: - Associate's Degree
- Associate's degreeKnowledge of current JCAHO, Title 22,Medicare
regulations pertaining to Home Health.BSN with PHN preferred.
